DSP-122: Just one form (the person who won the lottery=principal applicant=petitioner)
DS-230: The principal applicant and all family members who want to have a visa as well need to complete DS-230 so ONE FORM FOR EACH OF YOU

what sista_mai meant was, the main applicant, who is the lottery winner and also the petitioner fills the ds 122 alone. The ds 230 part I, the lottery winner fills it together with the husband or wife including all children.
Ds 230 part I, question numbers 14 to 16, you provide your husband or wife information.
Ds 230 part I, question number 29, you list all children here with their dates and places of birth, and their addresses. If the children are more than the space provided, you can list all the children on a seperate sheet with their dates and places of birth, and their addresses and write the question numbers of the ds 230 and attach it. I dont think so Bennit. From what I understand everyone got to fill one DS-230 EACH!
when the petitioner fill the DS-230, he has to list all his children, indeed, but sometimes not all the children you got will ask for a visa as well,
So this is why all the other people from your family who want to get the visa as well need to fill a form DS-230 for themselves, even if they are 1 years old,

Just follow what the forms says. The principal applicant (dv winner) and any family member intended to move to the US should complete the ds-230 both part I & II, while the main applicant only completes dsp-122. If you are finding it difficult just google for the instructions for filling those forms.
