Format to expedite RTD (please help)???

I have a letter from work which explains the urgency of my trip.
Could you please send me a cover letter format,for the motion to expedite RTD.
It is URGENT please help!!!!!!!!!

Send all the proof to Nebraska Service Center and make sure you write
"ATTN: Refugee Travel Document Supervisor/Director" above the address line. Otherwise your letter will be lost in the sea of documents and would reach an appropriate personel.

Include return envelop with first class postage.
 
Thank you ,Am I gone use the same address as the RTD address when I send it the first time??
