Format for FP letter and Fax number of TSC

Dear All

Could you please share with us the format of FP expedite letter and fax number of TSC ?
Hope it helps to send one ....( or 2 per month )
My ND/RD is Jul 2002 but no FP recd... seeing 2004 guys receiving FP (for concurrent filers) it is very frustrating
for 2002 filers.

Also if someone please guide how to contact congressman or senator for the same We are in Memphis TN
