Form I-485 Help Required

I need help filling out Form I-485 with the appropriate answers regarding some of the questions asked. Here's a quick background before the questions I need help with: I came to the US in 2008 from Canada with a TN visa. Switched to H4 in 2010, never undertook paid employment in the US, currently doing a Master's program.

Questions
  • Social Security #: Should I put in my ITIN or simply put 000-000-000?
  • Alien #? Not sure what this is. Is this the same as my I-94#?
  • Nonimmigrant Visa Number? My TD has no 'visa #' except for the I-94 #. I don't seem to have a visa # for the H4 too, except for the I-94 shown on my I-797A - Notice of Action
  • Consulate Where Visa Was Issued? Neither TN nor H4 was issued at a Consulate or Embassy - do I put in the border crossing where TD was issued?
  • In what status did you last enter?
 
Don't write the ITIN there, it is for actual SSN only. Leave the SSN blank if you never were issued an SSN.

If you never applied for a green card or asylum before, you wouldn't have an A# so leave it blank.

Leave the nonimmigrant visa number and consulate blank, since you didn't enter with a visa. But write the I-94 number in other questions where they specifically ask for the I-94.

The last time you entered the US, did you enter with the H4 or TN?
 
If you never left the US even once since entering with the TN visa and switching to H4, write TN for the status in which you last entered.
