Date arrived:

I came here on student visa then COS to work visa, how do we fill that part? I have 2 I-94s one is student and th e other one is work.

What does this mean? have you obtained a certif. of citizenship in your own name? My wife is USC born here.
 
I had same situation:

This is what worked for me:
F1- Student
I/94 # use student one, next to it put the arrival date
Date authorized stay will expire: "refer addendum" and on extra sheet mention both your I/94 and explain.

Where are you reading the last question from?

It's on the I-130 part B question 13... Do you have the "addendum" sample I can use?
