form i-130 question

Can someone guide me how to fill form i-130. I came to US on F1 and then after graduation I got a job and visa changed to H1B. My current I-94 reflects the last time I was admitted to US which is on H1B (not F1). My question is what should I write for arrived as? Should I write as student or H1B. The question is kinda confusing. First it asks for He or she arrived as and then asks for the Date of arrival.

I arrived as a student but my current I-94 arrival date is not the same as the arrival date as a student.:confused:

Thanks Triple!! You mean my current I-94 that is on H1 right. So I should write as Arrived as H1B.

One more question I have is:
#22.Your relative is in the United States and will apply for adjustment of status to that of a lawful permanent resident at the USCIS office in:

for the above should I write the DO or the sub office. I'm from Wisconsin, my DO is Chicago and sub office is Milwaukee. Appreciate the help :)
 
Thanks!! you are right triple. I found another thread which has a office locator and when I checked it in fact showed as Milwaukee :)

#16.Go to part 2 of the instructions, Eligibility categories. In the space below place the letter and number of the category you selected from the instructions.
Can you please let me know what this means. I'm not sure which eligibility category i come under. I'm on H1B as of now.
