Your relative is in the United States and will apply for adjustment of status to that of a lawful permanent resident at the USCIS office in:

will apply for a visa abroad at the American consular post in: _________ __________

What should I put in this area if anything? I am USC and my wife is from Jamiaca and she is here with me in Columbus Ohio.

In the first blanks we wrote our local DO and in the other we wrote N/A. I guess they were happy with that.

if you would want to do CP for your wife, you would need to file the US COnsular office in your wife's home country. otherwise, you could just put N/A to the "will apply for a visa abroad at the ......."

Can we fill both sections. I am currently here on H1 but my parents are US Citizens. If I stay in US I would like to adjust status but if i move to other country than what will happen Since in my category it is taking 8 yrs or so.

you have to pick one of them. If you are unable to go with that option later on, you wil file form I-824 (request to act on a petition) after your I-130 is approved, so that your I-130 is trasferred to the other route.
