form G 325 help. Is student and occupation?

For the name and address of employer, should i write the address that's on my W2 or the physical address of where I work. the reason for this question is that I used to work for a company in Texas, however, the address that shows on my W2 is the headquaters in New York. Which of these addresses do I use?

Another G-325 question. So applying for my husband we need to fill 2 G-325s.

Form1 - For Myself
Form2 - For my husband

My question is what goes in the big bold box at the bottom of the page. Will both Form1 and Form2 have my husband's name (as he is the applicant). OR the forms will have the respective person's name.?

The respective person's name. Both persons are applicants for I-130. You are the petitioner, he is the beneficiary.
