Form for extension of stay

Someone I know would like to extend their stay in US. The person got B2 Visa and I-94 approved for 6 months. What form do they need to fill out to extend the I-94? How long the proccess takes? Person still has 2 more months before I-94 runs out.
Will there be any consequences of re-entry to US if the person extends their stay?
