Form DS230 question - Urgent please

Hi,
In the Form DS230 part 1,
# 10 "Permanent address in the United States where you intend to live, if
known (street address including zip code). Include the name of a
person who currently lives there."

I am not currently in US. can i give my office address here or it's better to leave this blank ?

Also
#11 "Address in the United States where you want your Permanent
Resident Card (Green Card) mailed, if different from address in
item #10 (include the name of a person who currently lives there)"

can i give my office address here ?

Hi

I am in similar situation. Here's what the lawyer has instructed: in the permanent address box, write "Not Currently Known". In the box where the GC would be sent, I placed an address of a relative in the U.S. The lawyer said it has to be a U.S. address. This can be changed later by phone I think before they send you the GC.

One more question
In #25 where you have to give all the previous visits and residences, I visited USA in 1999 on H1b for around 1 year and lived at 4-5 locations during this time. Should i mention all my locations ?

If you can recall the dates/periods when you lived in a particular place, you can write each one. I just wrote the city and state, like Atlanta, Georgia. One time I was there on a tourist visa and visited several places so the lawyer just put "Tour of various cities". But then you're H1B then, so I think it's better to list all places you lived in during that time.
