Hello there

I am on H1 Visa, completed three years in US and working as software consultant; will appreciate if someone helps me with answers to following questions for Form D-156.

Q 19. What is the purpose of your trip?
Q 23. Who will pay for your trip?
Q 27. Have you ever been in the US?
Q 28. Have you ever been issued a US Visa?
Q 31. Has anyone ever filed an immigrant Visa petition on your
behalf?

Only you know the answers to these questions! How can anyone else possibly know the answers to 27, 28 and 31. You have already answered questions 27 and 28 in your query itself. Just state the truth. Update the forum with any experiences you face at the consulate.
 
As Raj1994 said you are the one who should know the answers to these questions !

Anyways, since you are on H1 here would be the appropriate responses:

Q 19. What is the purpose of your trip?
Ans. WORK or EMPLOYMENT

Q 23. Who will pay for your trip?
Ans. If you are paying then say 'I will pay' else if co. is paying say 'Employer will pay' (I should not be telling you these things, you should know them :)!!)

Q 27. Have you ever been in the US?
Ans YES (Ofcourse, you said you have been in US since last 3 years !!)

Q 28. Have you ever been issued a US Visa?
Ans YES (Ofcourse, you said you have been in US since last 3 years !!)

Q 31. Has anyone ever filed an immigrant Visa petition on your
behalf?
Ans. (You tell me... have you filed for GC ??)

These are simple questuions whats in it that you did not understand ??

My english is not very good(I am learning lot of languages Java, english etc), hence all these problems.
Well i haven't filed for GC, so my answer for question 31. would be "No".Right.
