form and other supporting doc for AP

Hi Everyone,
I have applied for I-140 and I-485 concurently on May 2003. I am planning to apply for AP now.

Can anyone please let me know what form(s) to fill and what supporting documents to provide for AP application.

Following is required:

A) Form I-131
B) Affidavit indicating the need for AP Authorization.
C) Two Photograph
D) Copy of 485 Receipt
E) Applicable fees.

I have another question.

What is the default for question 7.1 of the I-131 form?
The question is:

For how many trips do you intend to use this document?

options are: 1.One trip and 2. more than one trip.

I guess I can tick option 2.
