For those who got Medical RFEs.....

As I mentioned in my earlier thread, that RFEs have been issued for all of us (self, spouse, both kids)...

Today Just now I got the RFEs for my kids…It is the same for both kids for their medical tests.

USCIS returned their medical forms and the RFE letter says ...… "you need to return to the doctor who did the medical check-up to obtain a completed supplemental form to I693 to establish that you are current for all the age-specific immunization".


Although I see in the original Medical form (I693) , the doctor has signed off on 'Current on all age-specific immunizations'...there is nothing missing in earlier medical form. So, Frustrating....

My questions to those who got & handled the Medical RFEs....

1)What is the supplemental form to I693?
2)Do we have to get a new medical form filled out?
3)Does USCIS strictly follow that the doctor who does the medical test has to be a listed doc in INS website?
4)If the original doctor is not available, Can it be done by other doc?


I would appreciate your inputs on this ASAP....

Just doctor signed off on 'Current on all age-specific immunizations' on form I-693 is not enough. Your doctor has to append to I-693 a supplemental form that lists all finished immunizations, and your doctor has to sign that form also. This supplemental form is the evidence that establishes that you are current for all the age-specific immunization.

The doctor who did our medical check-up & filling of the I693 , is not listed on INS Civil Surgeon list.

Now , I need your advice on .....

1)Should I go the same doctor with the I693 Supplemental form to fill up?

or do you suggest to go to different doctor from the INS list of Civil Surgeon?

2)I693 Supplemental form is also needed for Adults? We donot remember the date of some of the immunizations given to me in my childhood.... So, How does the doctor generally fill out the I693 supplement form for me & spouse?

If you go to the same doctor, he should not charge you additional fee. You can also go to a different doctor as long as he is listed as INS Civil Surgeon. I693 Supplemental form is required for all applicants. As teh immunization date, some doctors will just ask you and fill the date. So think out a date by yourself.

I am sorry. I am not able to give a positive answer. I guess that you can do either: 1) trying to get a supplemental form from an INS approved doctor and asking your doctor to fill it out; 2) finding a INS approved doctor and redo you medical. I understand the cost if you redo the medical. However by doing (1) INS may send you another RFE.
