I am a US citizen and have a 30 year old sister who is currently living in USA with no status. She applied for 245i, then got DV, so she cancelled 245i, but unfortunately DV application could not be processed before the expiration date.

How can I apply for her? What is the form number?

You can file an I-130 for her in FB4, but it will give her no legal status and will take 10+ years for her priority date to become current. Unless 245i is reinstated she won't be able to adjust status anyways.

She is pretty much out of luck except via a US citizen spouse.
