For Info sake ( I-485 & I-140 link)

This is for information sake only :

we know that If your I-140 gets rejected , I-485 automatically gets rejected, now what if you have an approved I-140 and your I-485 is rejected ( say for some strange technical reason ) in that scenario does one have to refile I-140 and start the whole process from scratch or just refile I-485 while I-140 once approved remains live forever?

Rejected or denied...?

Rejected might be for wrong fees and so on. Denied might be for some serious reason. Please clarify?
 
Actually I meant rejected for lack of some paper etc. But my main query is iin both these circumstances can one apply new I-485 on the basis of the previously approved I-140 or does he have to start all over.

Also how can we tell to which I-140 ( if multiple applied) is the I-485 linked too. Can we tell it by the receipt number itself ( or the receipt number the same for I-140 & 485) or we can check it later by logging in to the uscis website?
