for cancellation indian passport , i have to add passport...
beside passport do i have to add any for like ronounce citizenship of india
?????
do u know guys how long it will take to get my document back???
cos i m goint to india on 5th july..

You don't need any document or form for canceling Indian passport - just mention in your cover letter that you are including your Indian passport for cancellation.

It shouldn't take more than 10-15 days to get your passports back - include a copy of your itinerary and mention in your cover letter your travel date and request that your passports be returned at their earliest convenience.

If you apply for any visa, simply send the Indian Passport along with your US one. You will get it back within a week.
