Where did you mention name check request on privacy Act request form?, How did FBI knew you sent FOIPA for I-485?, did your name check completed?

Sorry to hear that. I downloaded and filled FOIPA form, got it notarized. I faxed one copy and Mailed the original form using priority mail. That's all I did.

what is the fax number that you used? I couldn't find it on the FBI site. Also, I've read from other posts that you don't need to notarize the request form if it's for yourself. Did you send the form for youself?

Any help from anyone on this is apprieciated!!
