Can someone tell me how to write FOIPA? I got the form but it as only name and DOB,do I have towrite a separate letter with A# and Adress? does anyone have a sample letter for it. Also who should i adress To when i write a FOIA letter.

FOIPA just requires your name, date of birth and address which is what the form you have linked to asks for. FOIPA is meant for everyone, not for just the AOS applicants, so the A# doesn't have any meaning when filing it. If it makes you feel better, you may include a covering letter with other details about yourself that you may want to include, regardless, the FOIPA search will be based on your name and your DoB.
