Filling G-325A, Biograhic Information, Please Advise

Hi,

My name on passport is BABOO RALLY. It does not have my family name (due to a mistake earlier, my last name didnt appear on my school certificate and pasport was prepared using the name on the certificate.).

What do i fill under following item on G-325A

--------------------------------------------------------------
(Family Name) (First Name) (Middle Name)
--------------------------------------------------------------

Should i leave (Family Name) blank and put BABOO under (First Name) and RALLY under (Middle Name).

The other option of putting RALLY as (Family Name) does not appear correct to me, for two reasons: it is not my family name and it would not match my father's family name which is another entry that i need to fill in G-325A.

Please advise.

In my opinion, if things need to be correct before apply for a benefit, do so first and then fill out correctly. You shouldn't leave any (especially the part of your name) space in blank. They need your complete name including middle name.
Second, I would not put my real name over a forum in internet for security reasons. So use an alias or an example for that, just as a precaution.
