Fillable form G-325A. Anybody submitted this?

My attorney sent in G-325A forms to be filled up. I found it difficult to fill in as you have to press very hard for all four copies to be legible. I also made some mistakes. Anybody submitted the fillable forms available over the internet? Any problems encountered?

I did. I downloaded the form from (pdf file) internet and filled it up with acrobat exchange. It worked fine.
