I am also perparing for EAD renewal. The Documents form the INS web site is pretty clear.
You can download this I-795 from www.ins.usdoj.gov choose "Forms and Fees" link.
One thing is not clear is the fees. The Application instruction says it is $ 120 , I read/heard some where the fees has changed recently. Lets post the fees whoever finds it .
