Filing AP along with I-485

I am filing AP (I-131) application along with I-485.

1. Should I provide duplicate copy of required doc? There're some doc reuqired by both I-485 and I-131, such as I-94 and passport.

2. Do I need separate cover page for I-131?

3. Should I provide a separate sheet to explain the reason for foreign travel?

----- If lawyer is preparing the I-485, I-131, I-765 then provide the copy what he asks
