Filed wrong form,what should I do

Hi guys, I just noticed that I filed G-325 form instead of G-325A form back in Nov,2005.
I called NSC office, the rep said I should get a notice to file the correct one, but it has been 7 months since they received my G325 form(wrong one), my case has been stuck there since.

Should I get another Medical Exam and together with the correct G325A form?

I remember the doctor did these forms and I just pick up and sent it? why you both did by yourself?
