Fellowship Application for Lottery Winner, but GC not received...

Hello fellow residents/physicians.
My wife is one of those lucky winners of the DV2008 lottery, but unfortunately our case number is very high for the Europe region...so there are chances we might not get to the interview! I am currently an IM resident in my second year, on H1.
If anyone could clarify some things for me, would be much appreciated!
1. can i apply for a fellowship position this fall, saying i won't need a visa for my fellowship? ...thinking that we can get to the interview and get the GC by fall of my 3rd year!
What other possibilities are there?
Or, let's say i apply this year saying i will need visa sponsorship...if i don't get anything, can i apply in my 3rd year of residency to the same programs (this time with a green card)...or once they reject you, there's no reason to apply there again?...and with this, i will need to find a place for 1 year...as hospitalist or so.
What would you guys do if you were me?
BTW, i am thinking of applying to Nephrology.

I would just apply where ever you want. Visas are discussed once they decide they are going to take you. I won't rely on the GC as there is something called FBI name check which can be a nightmare.
