FBI Name Check and FOIA

Hi, everyone,
I have read so much about name check but I am still confused as to what to do. I went to the fbi website and wonder which document I need to use:
FOIA Request Form or Privacy Act Request Form

In addition, do I need to have the form notarized?

You need the Privacy Act request form from the following link: http://foia.fbi.gov/privacy_instruc.pdf
Include other informaiton like receipt#, prior addressess etc in a separate sheet. You DO NOT need to notorize it. You have to mail it after signing (don't fax it as they need original signature)

You don't need to notarize it, but is very important to include the statement bellow and sign it after that.

"Under penalty of perjury, I hereby declare that I am the person named above and I understand that any falsification of this statement is punishable under the provisions of Title 18, United States Code (U.S.C.), Section 1001 by a fine of not more than $10,000 or by imprisonment of not more than five years, or both; and that requesting or obtaining any record(s) under false pretenses is punishable under the provisions of Title 5, U.S.C., Section 552a(i)(3) as a misdemeanor and by a fine of not more than $5,000."
 
after two weeks, i got the reply from FBI regarding FOIA. as expected, i was informed that there is NO records regarding my investigation! what should i do now? call USCIS? contact my congressman?... please help. thanks!

You can contact your congressman but I doubt it will help. FOIPA and name check are 2 separate processes and are not related to each other. FOIPA only proves that you are not under any criminal investigation or are not a witness to any crime. Name check on the other hand traces various aspects of your name, nationality affiliation etc and is much lenghty process than FOIPA.

If your name is not clear in a year, you can sue USCIS. Details are in the above post.
