SIgnature card is a white sheet with blue boxes on whci an applicant has to sign his name within that blue box. Unfortunatelt this card cannot be downloaded from the internet. One can request INS to send it you by mail. I donot know whether local INS offices have thi scard handy.
******NOTE********* From my experience and of others, Vermont Service Center does NOT seem to require this signature card with the application.

If one does not receive the plastic card within 90 days of stamping, one can file Form G-731 with the service centers which handled their 485 applications. There is no fee for this form.

INS regulations make it MANDATORY that any change of address be intimated to INS within 10 days of the address change. One can do it via phone and/or via Form AR-11
