Extending parent's visitor Visa

Hi,
My parent's 10 Year multi entry visa is about the expire in August '10. When is the best time for us to request an extension (e.g., immediately, or after expiration, etc.)? Also, is it better the request the extension while my parents are in India or in US.

BTW: I am a US citizen and have been for last 4/5 years.

They cannot "extend" a visa; they must reapply and appear for stamping just as they did initially. If there is no evidence that they abused it (like spending am unusually large amount of time in the US), it should not be difficult to obtain a new one.

Since they must appear at the consulate, they must be at their home.
