Ext approved but validity date is way past

Sorry, i'm new here but i just need urgent help regarding B2 extension.

Here's the facts -

arrival in US: july 2007
i-94 expiry: january 2008
filed for B2 extension: december 2007 (based on valid medical emergency reason)
extension approved: just this august 2008 (notice received yesterday, aug 18)
validity date of extension on notice: "valid from 01/22/08 to 07/21/08"

so what should be done now, since the validity date of the extension and new I-94 is only until July 21 and we are well beyond that date? can a re-extension be filed?

If the medical emergency situation still subsists you could file a second extension but right now you have started accumulating "out of status" stay. So you have to move fast.

The visitor got six months more from the original I-94. Why didn't he or she leave in this period itself, irrespective of whether the I-539 was adjudicated or not?
