expired twice

I have a female child, 3 years old, she got a 3 months B visa, then it has expired without travelling to the united states of america, after that, i re-applied for her to take the visa again, she got also a 3 months visa, then it has also expired without travelling, i fear now that they refuse her if i re-apply for her for the 3rd time, is their any document to fill in this case or any procedure to take into consideration before going to the embassy ? and what is the possibility that they refuse her ?
 
why do you keep getting her a visa and not using it? There is nothing preventing you from applying again.

ps: female child = girl
 
Not using previously issued visas should not work against a third application.
However, unless there are concrete plans to travel, why are you wasting money in applying for these visas?
