Expired H4 Visa

Hi,
My spouse's h4 visa expired last month. My h1 was renewed and my spouse's name was not added to it.

Did anyone face this scenario.
Can anyone let me know the procedure in such scenarios.

you were on H1 and spouse on H1
your employer filed your H1 extension with USCIS but your spouse H4 extension was NOT filed.your spouse needed to file form I-539 with your H1 papers or should have filed for H4 extension BEFORE spouse I-94 Expiry date. if your spouse I-94 is expired then your spouse stay is illegal
