Expedite request

Some people were asking about how to send a letter to the TSC to ask for expedite case processing. Actually there are forms for each center to do that. Here is the link to the form used by TSC: http://uscis.gov/graphics/formsfee/forms/files/g-731t.pdf
If your FP is going to expire, you can use this form to send request. Good luck all!

Is this form used for I-485 case to be expedite processing?
I don't see anywhere in the form to indicate the case number.

This form is for BOTH I-551 and general inqury. There are two pages the first page (INS called page 2, which is confusing) is for I-551 and the second page (which should be the first page) is for general inqury. You will find places to input case number...etc. on this page.
