EVL sample letter needed

Hi all,

It seems everyone get REF for 485 these days, I want to prepare
all the documents in case I receive the REF, anyone here can forward me the sample for EVL?

I have a sample (but it's @ home and I'm @ work now).

However I can't seem to understand how you can prepare in advance, since your EVL needs to be of a current date.
