Entered US using visa after getting green card. Please help!

Please help!

My mom arrived today from India (point of entry Newark NJ).

She has a green card, which arrived while she was in India.
She did have her advance parole (I-512L) with her. (I had applied
for adjustment of status during her previous visit here. Then
she had to go back to India to attend to some business while
adjustment of status was pending. She went with an advance parole).

At the point of entry, she forgot to mention to the immigration
officer that she has a green card now. The officer did not ask
for any documents. He just saw the old valid visitor visa in her
passport, and processed her according to that. He gave her
an I-94 with duration of status of three months.

She was exhausted after a 15-hour flight, and speaks little English.
So, she did not understand that she is being allowed to enter
on her old visa.

I am worried now, and have a few questions:

1) Is this situation bad (entering using a visa after becoming LPR),
and will it have repercussions later on? Does the use of this visa
affect her PR status in any way?
2) How do I go about rectifying this situation?
Who do I contact in USCIS to tell them what happened?
Are there any forms I can file?
3) Does the D/S in I-94 mean that she will have to go back
after three months?

Any help is much appreciated!
