Empty FBI Fingerprinting Card. Where to get it ?

Hi All,
I have a question regarding the FBI Fingerprinting form. This is for my brother-in-law who lives in India and has applied for Canadian Immigration.
He has been asked to submit FBI clearance since he lived here in the US for more than a year before he moved back. For this purpose he has written to them to request the FinerPrinting form by mail. Its been over a month and they have not even sent the card yet. What are his options for getting the empty card?

1)Is there anyplace where I can buy for him an empty FBI FingrPrinting card and send it to him?
2)Can I go to my local police station and request for an empty card for him?
3)Do I have to get the card from FBI itself, or can I get it at an alternate source like the post office or DMV or a private fingerprinting agency?
4)Can I use a different card for fingerprinting? More specifically I was planning to buy this

Are cards like these accepted by CIC?

Check yellow pages. Some of the Notery do FingrPrinting for canada immigration. Go to them and buy one.
2)Can I go to my local police station and request for an empty card for him?
Some time local police station charge $10.00 for card and FP. But you can try.
3)Do I have to get the card from FBI itself, or can I get it at an alternate source like the post office or DMV or a private fingerprinting agency?
Wht not your brother in law send a mail to FBI from your address to get the card.
