employment and address question in n-400

i got my gc on oct 6, 2004. i am applying on july15,2009 for n-400.
does my employment history have to go back to july, 2004 or just 0ct 2004. the same doubt for residence address.

Kumar, you need to tell them about previous address and employer information going back 5 years from the day you fill the application.
