Electronic version of DS156

Did anybody filled out DS156 electronic version recently? I don't see any instructions other than blank text fields. Is it only me or anyone else having this issue.

Your help is appreciated.
 
I tried on my computer just to see and it displays everything for me. So u might want to try on another computer.
 
I had the same issue, HOWEVER, I found out that if you fill out the form during the work hours, you would see the entire file.

It is weird but worked for me.
 
Same issue. While I was trying to do it from home on sunday it didn't work. But when I opened the file on monday on office computer everthing worked fine.
