Elderly GC Holders-info

Hi,
I needed information on two aspects.
1.If an elderly couple over 60 years of age but less than 65 have recently immigrated to US. what options do they have to get medical insurance-from state/federal health schemes or fully paid big insurance companies like bluecross so as to cover all pre-existing medical conditions also.How much is the premium per month likely to be
2. There is a eligibilty requirement to become US citizen, the words as picked from the official document posted by BCIS are
An applicant is eligible to file if, immediately preceding the filing of the application, he or she:
· has been lawfully admitted for permanent residence (see preceding section);
· has resided continuously as a lawful permanent resident in the U.S. for at least 5 years prior to filing with absences from the United States totaling no more than one year;
· has been physically present in the United States for at least 30 months out of the previous five years (absences of more than six months but less than one year break the continuity of residence unless the applicant can establish that he or she did not abandon his or her residence during such period) · has resided within a state or district for at least three months

The confusion lies as in if there absences from US in not more than 6 months any of the times they travelled abroad then can they still file for US citizenship after 5 years from the first time they entered the country? Where does the condition of absences totalling no more than one year goes?
If somebody knows about this please advise..
All help will be really appreicated
Thanks!
