Effidavit of Support

Hi folks

I am new to this forum. I very much appriciate the information provided in this forum.

I am a DV-2008 winner. My interview is in April.

I have an urgent question about Effidavit of support. My sponsor in USA has asked me following questions:

1) Is form I-134 is the right form to fill and is it the only form that he needs to provide?
2) Should he (my sponsor) send the completed effidavit of support to me in my country of residence or he has to only lodge it with the immigration department in USA?

Please reply as soon as possible.
 
He needs to fill it out, sign and notarize it, enclose all of the supporting documents listed in the instructions, and send it to you so you could bring it to the interview.
