EB1EA I-140 via PP: would you apply concurrently for I-485?

HI guys,
I am trying to get some opinions and experience together on the following issue:
if you file your I-140 for EB1-EA with PP, would you concurrently for I-485?
Or would you just wait to hear about the decision before you submit your I-485 application?

Does it make much of a difference in terms of getting your I-485 through quicker?

So, you mean 2-3 weeks after your I-140 was approved and you do not file your I-485 concurrently?
I thought it may take much longer, and filing concurrently may speed up the transition for them to look at your I-485.

medical exam may expire

I heard that if I140 is not approved within 6 months, the medical exam concurrently filed with I485 will expire. Is that true?
