EB-2 NIW Rejection

I Applied the EB2 Niw Premium process. But my application was rejected. I have to reapply. Can you share some solutions? Please see my attached files. I didn't understand multiple forms not acceptable. I use only I-140 , I-907 , and I-1145 (for notification ) .
May i know the reason ?
