EAD replacement

Hi Guys,

I have misplaced the file which contained my 140/485 receipts and also my EAD card. How do I get a EAD replacement. The I-765 form has an option for EAD replacement but I have no supporting document proving that my EAD was approved. How do I go about this. I want to apply this on my own since the lawyer is asking for $500 to apply for a replacement.

------- If you can get copy of I-485 receipt then it has your A # on it, you can file for new EAD with I-485 receipt and explain in the cover letter to BCIS that you lost the EAD Card and unable to provide the copy of both sides of past EAD or top portion. You can submit an affidavit that you have lost the XYZ documents. Write your complete name, DOB, SSN, your passport # attach copy of any past approved petition or past I-94 or H1 approval, copy of I-140 filing receipt or approved I-140 BCIS may check your record in BCIS computer
