EAD renewal question

I will have to renewal EAD myself. The lawer of my company did it for me for the last time renewal. This time I have to do it myself. I decide to mail the renewal application.

Questions:

Which documents should I prepare to file the application?

Which forms?

Does it need to attach any employment document ?
 
Form I-765
BCIS Fee Check $120
Photographs (2)
I-485 Receipt
Copy of I-140 approval or BCIS Online print out
Top portion that you got with EAD approval notice
Copy of EAD (front and back)
I-94 copy if you have
employement letter not needed
Write out side of envelope in Bold” Renewal of EAD”
 
also if you don't want to send application

you can file online which would save your couple of bugs for photographs.And then u can send additional documents as ginnu if required.
