ead renewal question

I am applying for EAD renewal myself. I have a question filling out the form
In the section where they have asked for date(s) besides "which INS Office" and above "Results (Granted or Denied)" , what dates should i enter. Are they current EAD validity dates?

Is it OK to apply EAD renewal oneself and not use an attorney?

But U can get one from your local INS office.
