EAD renewal online...help

Please advice. I am going to file online for the renewal of EAD. Below are my GC application details.

- Original H1B is valid up to May 2003.
- Applied for I-140, I-485 and EAD in Dec 2002 from same company.
- Received EAD in Jan 2003 and valid up to Jan 2004.
- In Feb 2003 went to India and returned on H1B. I-94 is valid up to May 2003.
- In May 2003 applied for renewal of H1B and received approval with new I-94 attached and valid up to 2005.
- Continuing the work with H1B status because I-140 approval is pending.

Now my questions are
1. Which one I should select on form I-765? A# or I-94? If I-94 then which I-94 number should I write? Last US entered in Feb 2003 or newly received I-94 with H1B approval?

2. I do not have previously EAD application date. My lawyer applied previously. Any information on original received EAD (I-797D)?

Thanks and finger print question

Thanks for your suggestions... I applied online.

I got to go for finger prints. Recently I sent my passport and I-94 for H1B visa revalidation to St.Lois. At the time of finger prints does the Application Support Center ask for original passport or I-94? I do have photo copies. Is it OK to show photo copies and explain them the situation?

No need of passport, 2 copies of all receipt notices from online filing and driver ID or previous EAD are enough.
