Folks,
My 1st EAD got expired and I still haven't received my 2nd EAD. What I did is that I went to the local (district) INS office, when my EAD was just about to expire. Showed them a letter from my employer, got an appointment for the interim EAD. Showed up on the date of appointment and got the (roughly) 3 months valid interim EAD. They basically keep your old EAD and ask you for a picture ID.
Very simple process and you don't have to wait for 3 months/ 90 days if your EAD is expiring BEFORE that date.
