EAD Renewal and AP first Time

Hi,
Applying for EAD Renewal and also Advance payroll.
I haven’t used my EAD, is it any problem? My spouse was using her EAD.
Now we are planning to apply for AP also (first Time).
What documents I have to submit with AP?
For EAD, I am submitting below documents…………
•Check payable to BCIS for $120
•Form I-765: Application for Employment Authorization.
•Copy of i-485 receipt notice.
•Copy of Photo Identification Documents ( Employment Authorization Card).
•Copy of I – 94.
•Copy of Approval Notice & EAD.
•Two photographs.
and I got I-140 Approved, should I submit that also.
what is the address I have to send the forms.
I got the address : 75 Lower Weiden Street ,
St. Albans, VT - 05479 -- Is it correct?

First off-It is advance Parole- as in parole from a jail.
