EAD for myself

I just got EAD approved for my wife and now I want to have for my own. My lawyer applied for my wife's EAD. This time I want to file my EAD all by myself

Please help me to file by myself. I prefer to mail all the documents instead of submitting it electronically.

Please give me the instructions in detail as to:

1. What documents I MUST submit/enclose without receiving RFE
2. Should I have to send 2 PHOTOS
3. What is the mailing address for Vermont?
4. Are there any special precautions I should take?

And anything that I haven't covered...
