Ead documents needed

Hi

Finally I confirmed that Boston is nolonger giving walk-in EAD's.

Can anybody give a checklist of document that need to send to INS service center(VSC) for new EAD application?.

Can I send a personal check of $88 for fee?.

2 ADIT photos(with Name, A# and 485 receipt number penciled at the back)
personal check
covering letter
Find the PO box from the Service center web site
