EAD, AP renewal checklist. ginnu help

I know there is already sticky thread for but that didnt look up-to-date.

Can someone forward me up-to-date link containing checklist of PAPER BASED EAD, AP renewal? My I-485 is pending with VSC, which address should I send renewal applications?

check the sticky it has address and documents to send or write what you are going to send and if miss something that will be corrected by me or others
 
EAD
Check in the amount of $180 for the filing fee;
• Form I-765 with two photographs;
• Copy of I-485 filing receipt notice;
• Copy of I-140 approval notice;
• Copy of front and back of I-94 arrival/departure card and latest H1B approval notice;
• Copy of front/back of last EAD and previous EAD mailer;
• Color Copy of driver’s license;
• Color copy of passport’s biographic page;
• Copy of Birth certificate.
AP:
• Check in the amount of $170 for the filing fee;
• Form I-131 with two photographs;
• Part 7 supplemental information;
• Copy of I-485 filing receipt notice;
• Copy of I-140 approval notice;
• Copy of front and back of I-94 arrival/departure card and latest H1B approval notice;
• Copy of last approved Advance Parole document;
• Color Copy of driver’s license;
• Color copy of passport’s biographic page;
• Copy of Birth certificate.

CAUSA: thanks a bunch for passing check lists.

Ginnu, CAUSA and others; Should I send original of mailer or copy of it. I will use this checklist. let me know if anything is missing. what should I put under this on I-765 form?
"Eligibility under 8 CFR 274a.12 : " ? would it be (c)(9) (both in seperate bracket space)? can you please conform this.

Also my I-485 is pending at VSC so I should mail these both renewal applications at following address. Can you please confirm this too?

*** for dependent EAD or AP also send copy of marriage certificatecan you please confirm, may be last question on EAD renewal.

For EAD/AP renewal paper based only:

1) i am working on EAD and used AP for travelling. So, no need to attach the H1B copy, correct?

2) I should mention that i am working on EAD in Part 7Supplement letter for AP?

3) also, is it ok to send the fax copy of I-485 receipt notice for EAD/AP renewal? Because my attorney faxed me the I-485 receipt notice.
 
Ginnu, please help

1) My wife was originally admitted into the US as H-4. But now after enetring US on H4 and staying on h-4 for about 6 motnhs she used her EAD to work. Her H4 visa is still valid even though our Visa stamping on the passport has expired. So what should her class of admission be ? Should I write H4 or EAD ? AP form asks for class of admission



2) Part 7 Info for me asks only if my trip is for single entry or multiple entry. Id does not ask ask for any supplemental Info. What is this supplememntal info peopel are talking about ?



3) Part 3 asks for processing Info.


"Are you, or any person included in this application, now in exclusion or deportation proceedings? "

If no, then we DONT have to give the office name where the proceedings are pending ?

Then how will the system tell me at the end of filing which office i should mail the supporting documents to ?
