--------- go to USCIS main page and then go to NSC service center page and note the address for form I-131(AP) and form I-765(EAD).
* download the form I-131 and I-765 read the instructions and then post what you dont understand.

for I-131(AP) you have to fill up the form ,attach the fee check, 2 Photographs, copy of I-485 recept, copy of Passport pages that have your name, DOB, Photograph(send Enlarged pages or Color copy), Copy of Driving licence(Color copy), copy of I-94 (front and back) TOP mailer that you got with your first AP, you can also attach copy of the past AP. Write on the cover letter your name, DOB, A#, I-485 reciept # address and write what is attached, sign the letter with date.
